- School Address (drop-off location)
- Days needed (Mon–Fri)
- Pickup time (morning) + drop-off time (afternoon)
- Number of students
- Special instructions (guardian handoff, gate code, etc.)
Safety note: Please include who the student may be released to (parent/guardian).
